In-Home Assessment

The right first step when you’re not sure what care your loved one needs.

For the family, it is very difficult to know what type of care their loved one will need and whether home health care can be provided at home. In such cases, it is important to get an In-Home Assessment. The purpose of an In-Home Assessment is to:

  • Meet the family
  • Understand the wishes, wants and goals of the family
  • Get a better understanding of the patient’s medical history and co-morbidities
  • Answer any questions you may have about the agency
  • Discuss the options of care, including finances and services available
  • Develop a care plan and schedule visits
  • Assess the home for safety, for both the patient and the provider of health care

Once the assessment is complete:

  • You will be given a matching caregiver who will help meet your expectations and preferences.
  • A goal-oriented plan will be developed and discussed with you or the family.
  • The agency supervisor will come to your home with the caregiver on the first day to further assess the situation and form a working relationship.
  • You will be given a chance to further evaluate us after a few days to ensure that the relationship is mutually acceptable.
